Registration and Liability Waiver
To register as a competitor for SCPYWO Sanctioned
Tournaments, Scrimmages, Clinics, or other events, please complete this form
and return it to your Coach, who will forward it to SCPYWO. Wrestlers will not
be eligible to enter if this form (or a facsimile) is not properly completed,
signed, and in SCPYWO¹s possession prior to the event. Please note that bulk
team registrations for sanctioned events will be done by your coach, and you
will need to notify your coach if you are unable to attend an event in which
you are entered. Your team will be billed the full entry fee if you fail to
show or cancel less than three days before an event.
I, my parents (or guardian), and family recognize that Wrestling is a vigorous
contact sport, and that injuries can and do occur. In consideration of your
acceptance of my entry, I hereby release the South Central Pennsylvania Youth
Wrestling Organization, its sponsors, Member Programs, and participating school
districts, Coaches and Referees from all liabilities, claims, or rights to
damages or injuries or losses suffered by me directly or indirectly in training
for, traveling to or from, and participating in SCPYWO sponsored activities.
This completed and
signed form should be returned to your Coach, who will forward it to SCPYWO.
